What you’re experiencing has a name: alcohol withdrawal tactile hallucinations. It is a recognized medical symptom of withdrawal, and the specific type most people describe, that unmistakable feeling of bugs crawling on the skin, is called formication.

It is frightening. It is disorienting. And it is a sign that your body is going through something that needs professional medical attention, not something to try to push through alone at home.

What Is Formication?

Formication is the medical term for the sensation of insects crawling on, under, or through the skin, a tactile hallucination with no physical cause. The name comes from the Latin word formica, meaning ant.

It is classified as a tactile hallucination, meaning it is a physical sensation that feels entirely real but is generated by the brain, not by anything actually touching the skin. Unlike visual hallucinations (seeing things that aren’t there) or auditory hallucinations (hearing voices or sounds), formication is something you feel, and because it feels so physically real, it can be especially distressing and confusing.

People describe the sensation in different ways: bugs crawling on the skin, worms moving under the surface, an itching or prickling that won’t go away no matter how much they scratch, a feeling that something is alive inside them. In severe cases, people scratch and pick at their skin so intensely that they cause significant physical damage, open sores, bleeding, and scarring.

What Causes the Bugs Crawling Sensation During Alcohol Withdrawal?

To understand why alcohol withdrawal can produce this symptom, it helps to understand what alcohol does to the nervous system over time.

Alcohol is a powerful central nervous system depressant. With long-term heavy use, the brain compensates for alcohol’s suppressive effects by increasing neurological activity, essentially turning up the volume across the entire nervous system to maintain balance. When alcohol is suddenly removed, that neurological hyperactivity has nothing to counteract it. The brain and nervous system go into a state of extreme overactivation.

This overactivation doesn’t just produce shaking, sweating, and anxiety. In severe cases, it disrupts the brain’s sensory processing, causing it to generate experiences of sound, sight, and touch that have no basis in external reality. Tactile hallucinations like formication are one expression of this severe neurological disruption.

The crawling sensation is your overactive nervous system misfiring, sending signals to the sensory cortex that register as physical touch even though nothing is actually there.

When Does Formication Occur During Alcohol Withdrawal?

Tactile hallucinations, including formication, are generally associated with the more severe end of the alcohol withdrawal spectrum. They are most commonly associated with two specific withdrawal phenomena:

Alcoholic Hallucinosis

Alcoholic hallucinosis refers to hallucinations, auditory, visual, or tactile, that occur during alcohol withdrawal while the person is otherwise conscious and relatively oriented. Unlike the full confusion of delirium tremens, a person experiencing alcoholic hallucinosis may be lucid enough to carry on a conversation, but is simultaneously experiencing hallucinations that feel completely real to them.

Tactile hallucinations like formication can appear during this phase, typically beginning within the first 24 to 48 hours after the last drink. They can last for several days.

Delirium Tremens (DTs)

Formication is also a recognized symptom of delirium tremens, the most severe and medically dangerous form of alcohol withdrawal. DTs typically develop between 48 and 72 hours after the last drink and involve extreme confusion and disorientation, severe agitation, dangerous spikes in blood pressure and heart rate, high fever, and hallucinations across multiple senses, including the feeling of bugs crawling on the skin.

Delirium tremens is a medical emergency. If you or someone you love is experiencing confusion, extreme agitation, high fever, and sensations of bugs crawling on the skin simultaneously, call 911 immediately.

Who Is at Risk for Tactile Hallucinations During Alcohol Withdrawal?

Not everyone who goes through alcohol withdrawal will experience formication or other hallucinations. These symptoms are more common in people with:

  • A long history of heavy, daily alcohol use
  • Prior episodes of severe alcohol withdrawal
  • A history of alcoholic hallucinosis or delirium tremens
  • Poor overall health or nutritional deficiencies related to long-term alcohol use
  • Co-occurring mental health conditions

The kindling effect, where each alcohol withdrawal episode becomes progressively more severe, means that people who have experienced hallucinations during previous attempts to quit are at higher risk of experiencing them again, and potentially more intensely.

Is Feeling Bugs Crawling on You Dangerous?

The sensation of formication itself is not directly life-threatening, but it is a serious warning sign that should never be ignored. Here’s why:

It signals severe withdrawal. Tactile hallucinations indicate that withdrawal has progressed to a significant level of neurological disruption. This level of withdrawal can escalate to delirium tremens, which is potentially fatal without immediate treatment.

It carries real physical risks. The intense itching and crawling sensation drives many people to scratch, pick, and gouge at their skin. This self-injury can cause serious infections and significant physical damage.

It is psychologically overwhelming. The experience of feeling bugs crawling on you, especially when you know intellectually that nothing is there, is deeply disorienting and frightening. Without professional support, this psychological distress significantly increases the risk of relapse.

It is a sign the nervous system needs immediate stabilization. The neurological processes causing formication are the same ones that can cause seizures and cardiovascular instability. Seeking medical help quickly is essential.

What Does Medical Treatment Look Like for Alcohol Withdrawal Hallucinations?

The good news is that alcohol withdrawal hallucinations, including formication, respond well to medical treatment. In a properly supervised detox setting, these symptoms can be managed effectively, and the most dangerous withdrawal complications can be prevented.

Benzodiazepines

Medications like Valium and Ativan are the gold-standard treatment for severe alcohol withdrawal. They work by calming the overactive nervous system, directly addressing the neurological hyperactivity that is causing hallucinations, shaking, seizure risk, and cardiovascular instability. Properly dosed benzodiazepines can reduce the severity of tactile hallucinations and in many cases prevent the escalation to delirium tremens.

Antipsychotic Medications

In some cases, antipsychotic medications may be used alongside benzodiazepines to specifically address hallucinations and psychotic symptoms during severe withdrawal.

Nutritional Support

Long-term heavy alcohol use depletes essential nutrients, including thiamine (Vitamin B1) and magnesium, that play important roles in neurological function. Replenishing these is a standard and important part of managing severe alcohol withdrawal.

Continuous Medical Monitoring

In a medically supervised setting, withdrawal severity is assessed regularly using clinical tools. This allows the medical team to catch any escalation, including the development of more severe hallucinations or signs of delirium tremens, in real time and respond immediately.
